Post Stroke Care

A stroke is a life-altering experience and a leading cause of disability worldwide. We are here to help you navigate life after the event. Our continuum of care pathway combines up-to-date information with a personalized treatment plan dedicated to improving your quality of life. According to the American Heart Association/American Stroke Association (AHA/ASA), patients who follow a comprehensive and multifaceted care plan after a stroke have the best opportunity to reach their full potential.

The benefit of care coordination

All care plans are developed and overseen by one of our board-certified Physiatrists. The Physiatrist often works in conjunction with a psychologist and/or a neuropsychologist to develop your personalized plan of care. These medical professionals are uniquely qualified to care for and treat patients who choose to participate in ongoing rehabilitation after a stroke.

Our Physiatrist is in constant communication with experts in other disciplines so all of your post stroke care is integrated effectively. We may refer you to one of our trusted partners for some aspects of your care plan, including speech therapy, physical therapy and occupational therapy. Our care plans aim to improve your quality of life after a stroke. Many of our patients who remain committed to their program are able to return to work and/or driving.

A stroke may dramatically impact nearly every aspect of your life. Activities that once were routine may now be difficult, but we are here to help guide you along your journey to recovery. Our Physiatrists evaluate, manage and treat several conditions associated with stroke, which include but are not limited to:

• Mobility impairment (irregular walking or movement pattern) 
• Spasticity
• Cognitive impairment (difficulty concentrating, forming thoughts or remembering)
• Mood disorders (depression and anxiety)
• Bowel and bladder disorders
• Difficulty swallowing
• Bracing, orthotic and wheelchair needs
• Unexplained pain
• Return to work
• Return to driving
